Community Palliative Care Team
The Medical and Nursing team is the patient’s first contact with hospice, providing information on the services available to patients and their families. The aim is to support people to stay in their own home as much as possible if that is their wish.
This team maintains a constant and close working relationship with other health professionals involved in the patient’s care.
They also provide an out-patient service, make home visits to meet those who have been referred, and to conduct follow up appointments.
After hours telephone support and advice is provided by the Registered Nurse on duty in the In-Patient Unit 24/7.
In-Patient Unit
If needed, patients can be admitted to the six bed In-Patient Unit which has a home-like atmosphere where family and friends are always welcome. This is a short stay unit where patients are admitted for:
- Assessment and management of pain and other sumptoms.
- Specialised palliative respite care for patients.
- End-of-life care.
Our team strives to create a safe, friendly environment which tends not to be too structured with routine.
Patient and Family Support team
This team works as part of the wider hospice team to provide emotional and practical support to patients and their families throughout their journey with hospice.
Services include:
- Grief and loss counselling from qualified Counsellors and Social Workers. This is offered to the patient as well as family members from the beginning of their journey at hospice up to two years post bereavement.
- Bereavement support for family members: One on one counselling either at home, at hospice or via phone calls. There is a 7-week ‘The Journey through Grief’ course run twice yearly.
- A remembrance service is held every three months at hospice for family members to remember their loved one.
- Spiritual support is provided to inpatients and their families and those in the community upon request.
- Day Program at hospice offers activities one day a week for patients allowing a break for the family member caring for the patient.
- Living Legacies is a biography service whereby the patient and a trained volunteer work together to record meaningful memories and life stories. This is treasured by families.
- A Caregivers course for those people caring for loved ones to enable them to understand and be better equipped to care for this person along their hospice journey.
